COBRA FAQ’s:
- I received a COBRA letter that said my health insurance ended prior to September 30th. Why did it end? The COBRA letter is your notification that your group health coverage ended on the day you were Laid-off. Hynix is picking up your COBRA premiums through September 30th. COBRA plans are a continuation of the current group health plans. All employees and family members whose group health coverage was terminated because of the layoff will be transferred to the COBRA plans. If you want to continue with COBRA after Hynix stops paying for the coverage on September 30th, please note that the COBRA Election Form from the COBRA packet needs to be returned within 60 days of your layoff date as indicated in the COBRA letter.
- What do I pay for coverage?
Current monthly COBRA rates (1/1/08-12/31/08):
- Medical
- Single - $396.11
- Family - $1,069.51
- Dental
- Single - $38.73
- Family - $100.23
- Vision

- Family - $13.18

- When are payments due? Payments are due the First of each month. There is a 30-day grace period.
- Will I receive a monthly Invoice? No. You will not be sent an invoice requesting the COBRA premium.
- Do I have to purchase the whole package? No. You can elect one or all of the plans you are eligible to continue.
- I don’t need coverage, but my spouse does. Can they elect coverage on their own? Yes. Each qualified beneficiary has the same election rights.
- Where do I mail my COBRA payments? Please mail COBRA payments to:
Hynix Semiconductor Manufacturing America
Attn: Benefits
1830 Willow Creek Circle
Eugene, OR 97402
- Will these plans change? Yes. As far as we know, these plans will remain the same through December 31, 2008. Premiums and Coverage details will change effective January 1, 2009. All COBRA participants will be notified of any changes when Hynix gets those details.
